Kindergarten Registration Opens Today

Parents and guardians can sign up 5-year-olds.

Madison public schools will hold registration for kindergarten-age children Jan. 18, 19, 20 and 21 , 2011. 

Madison children who will be five years old as of Oct. 1, 2011 are eligible to register for kindergarten for the 2011-12 school year.

Parents are asked to obtain and complete registration forms prior to the registration appointment.  Forms are available on the Madison Public Schools Web site, www.madisonpublicschools.org under District Information or in school offices.

The district requires that the following items must be brought to the registration appointment:

  • Child's original birth certificate showing that he/she is five years old on or before Oc. 1, 2011;
  • Completed registration form;
  • Proof of residency: Standard Residency Form (4 pages) plus additional proof of residency documentation listed on form.
  • Immunization records for measles, mumps, rubella, DPT, polio (including one dose of DPT and polio vaccine on or after the child's 4th birthday), Varicella, Hepatitis B and HIB vaccinations.
