At 4:30 p.m. on July 8, Chatham Borough Officer Daryle Kelly responded to a two-car accident on Main Street near Brooklake Road, police said.
His investigation revealed that Julie Kravetz, 41, of Springfield was driving on Main Street when she was struck from behind by a 2011 Jeep driven by a 17-year-old male from Madison, police said.
Both cars were driven from the scene and the juvenile was issued a summons for careless driving, police said.
